I was able to copy the legal letterhead of one of the heavy hitting medical malpractice firms in my area off the internet who have nonstop commercials on tv unto an envelope which I addressed to one of the docs who I knew was working that day, complete with recycled certified mail and postage insignia and I stuffed it with about 15 sheets of paper and stuck it into my colleagues mailbox at work. When they opened it, it had “April Fools Day” in large font on the first page. Then I waited when I knew they would be showing up to work in our office where our hospital mailboxes are. Their hands were visibly trembling as they opened it and then I ran for my life. This particular doc got me good the prior year so it was payback time.

I hurt my back the first time at 25. Was physically fit, lifted weights, ran marathons. Had an annular tear later diagnosed by MRI but for a while I had no clue what happened or even what to do. Bed ridden for days. Could barely manage to toilet myself from the pain of compression due to sitting on the toilet. The initial pain apparently subsided but I dealt with lingering 7/8 radiating back pain for months later diagnosed as sacroiliac joint dysfunction by a DPT. Rehab helped tremendously. I'm still dealing with it today and I've developed levoscoliosis because of it but the pain is "only" about a 1 or 2 most days and I've back to working out the last two years without a hitch and being functional.

The thoughts going through my head at that time were the darkest I've had in a while and I had a lot of ideation as a teenager. I ballooned up in weight, took NSAIDs non stop and started drinking too. Didn't know what the point of the next 70+ years would be living like that. Idk how I could/would cope with a permanent paraplegia/quadraplegia. My condolences to anyone fighting that battle.

I imagine people in the medical field have a far more complex understanding of the limits existing in modern medicine to treat chronic pain. That knowledge probably adds even more weight to the sense of hopelessness one might carry with any chronic pain condition. I’m not in the medical field, but I saw your post. I know human language is just as limited, and “I’m sorry for your loss,” doesn’t fully capture your pain. You guys are already so emotionally and physically tapped out from the pandemic. I won’t pretend to know how you must feel right now, but I hope you can take time for yourself to process this loss, when the waves of grief reach your shores.

It's stories like this that make me hope that we move away from toxic cultures of sado-masochism. We never know when tragedy will strike or, hell, maybe even just a bad day on a string of bad ones can send some spiraling. We should be more forgiving of our students, trainees, colleagues, staff, and ourselves.

Get a new toilet brush just like the ones your hospital uses. Put it in the dishwasher of your kitchenette at work and when the person unloading the dishwasher freaks out, just insist that it is standard practice of the departement 'for at least 7 years or so' and it is getting done every three weeks.

I’m NAD, but am fully disabled and the struggle to find purpose in life and defining my worth outside of “productivity” was one of my greatest challenges. Unfortunately, our culture is very heavily skewed toward defining a person’s worth based on their work and not just their personhood. I’m so sorry your friend felt so tortured. My condolences to you and all who loved him.

To my fellow colleagues in medicine: what can us shrinks do to support you more? I've been considering carving out more of a niche and working with physician colleagues struggling with mental disorders, as I think there are so many of us that are lurking in the shadows and suffering. What could I do to make it easier for you to to reach out to us psychiatrists for help?
